Dear Mr. Maynard:

It gives me great pleasure to congratulate you upon being awarded the Distinguished Service Award of the American Speech-Language-Hearing Association (ASHA).   Your service to ASHA is most deserving of this recognition. Kathleen Cox, the sponsor of your nomination, will receive a copy of this letter and is asked to notify the co-sponsors of your election.

Those elected to receive awards by the Association will be recognized formally with a presentation at the Awards Ceremony on Friday, November 19, 2010 at the ASHA Convention in Philadelphia, PA.   As we draw closer to the Convention, you will receive further instructions and information from the Convention and Meetings office regarding rehearsals, request for a photograph of yourself, and other logistical information.   It is very important for you to adhere to the deadlines for requested information since the time needed to print your certificate and prepare information for publication is limited.   If you are unable to attend the Convention this year, we will be happy to mail the certificate to you after the Convention.

On behalf of the Committee on Honors, I wish to express my sincere congratulations to you.

Sincerely,

Doreen Oyadomari
